The Geriatrics and Transitional Care program at Lahey Hospital & Medical Center provides comprehensive care for individuals over the age of 65. We facilitate transitions across all levels of care, including hospital, skilled nursing (SNF), acute rehabilitation, long-term acute and home/community care, all with a focus on quality, safety and wellness.
Our providers hold clinical interests in skilled nursing settings for post-acute and long-term care.
