Access Your Medical Records
As part of your rights as a patient, you can access your medical records at any time. Lahey Clinic makes it as easy as possible to request and obtain your records at a nominal fee. If you have a patient portal account, you can view your medical records online.
How to Request Your Medical Records
To process requests for medical records, please download and complete the Authorization for Release of Medical Information form or send us a signed letter with the following information:
- Your name
- Address
- Date of birth
- Telephone number
- Lahey patient number (if known)
- Purpose of the request
- Type of information requested
- Where to send the medical record
- Your signature
Fees
If you are requesting a copy of your medical record from Lahey, we reserve the right to charge a reasonable cost-based fee for producing and mailing the copies, pursuant to HIPAA 45 CFR, 164.524. This rate depends on the size and delivery method of the records being requested. At no time will the cost-based fees exceed Massachusetts law (MGL Chapter 111; Section 70).
