Community Health Needs Assessment (CHNA)
Community Health Priorities and Implementation Strategy
Every three years, Lahey Hospital & Medical Center (Lahey) conducts a Community Health Needs Assessment (CHNA) to identify and prioritize the most significant health needs in the communities we serve. This assessment helps guide our community benefit programs, partnerships, and investments, and ensures alignment with federal requirements for nonprofit hospitals.
The CHNA is informed by a comprehensive review of quantitative data, direct community input, and collaboration with local organizations and public health partners. Together, these insights help Lahey focus resources where they can have the greatest impact on health equity and community well-being.

Explore the findings of Lahey's Community Health Needs Assessment, including priority health areas, populations served, available resources, and implementation plans.

Our health equity priority areas include: equitable access to care, social determinants of health, mental health and substance use, and complex and chronic conditions.
As part of the Community Health Needs Assessment, we focus on groups that may face greater barriers to good health:
- Youth
- Older adults
- Low resource populations
- LGBTQIA+
- Racially, ethnically, and linguistically diverse populations
These reports share the most recent health needs identified through the Community Health Needs Assessment and outline the actions planned over a three-year period to address them.
The Community Resource Guide was developed to highlight health and social service resources available to the community. As a disclaimer, the listings within this guide are designed as informational and are not to be interpreted as recommendations or endorsements.
