Effect of Multi-disciplinary Cancer Survivorship Clinic (MDSC) on Following American Cancer Society (ACS) Guidelines, Physical Activity, Nutrition and Quality of Life in Cancer Survivors

The purpose of this study is to test the effectiveness of the Multi-disciplinary Survivorship Clinic in helping patients to follow the American Cancer Society recommendations for cancer survivors.
