Personalized support to help you prevent, manage and understand diabetes

Diabetes Care That Supports You

Lahey Clinic provides care for people with type 1, type 2, and gestational diabetes, as well as prediabetes and related conditions. We offer care in both outpatient and hospital settings, with a focus on helping you manage diabetes safely and confidently each day.

What Is Diabetes?

Diabetes is a condition that affects how your body uses blood sugar (glucose). It happens when your body does not make enough insulin or cannot use it effectively, leading to higher glucose levels in the blood.

This can affect energy, healing, vision, and long-term health. With the right support and day-to-day management, most people with diabetes live healthy, active lives.

Expert, Team-Based Diabetes Care

Your care is led by expert endocrinology physicians and advanced practitioners, including nurse practitioners and physician assistants. Our team uses the latest diabetes technology and advanced management strategies to create a plan tailored to your needs.

We offer:

  • Personalized medication and insulin plans
  • Continuous glucose monitoring (CGM)
  • Insulin pumps and automated insulin delivery (AID) systems
  • Remote patient monitoring programs
  • Nutrition and lifestyle support
  • Coordination with other specialists as needed

Our goal is to provide safe, coordinated care that supports your long-term health.

Diabetes in Pregnancy

Our diabetes in pregnancy program provides specialized care for gestational diabetes and for people with type 1 or type 2 diabetes before, during, and after pregnancy.

We offer structured support, diabetes education, and coordinated care with your obstetric team, including use of CGM and automated insulin delivery systems when appropriate. Our goal is to support safe, healthy outcomes for both parent and baby.

Diabetes Education and Support

As part of our comprehensive diabetes care services, Lahey Clinic offers a diabetes education program recognized by the American Diabetes Association. The program provides education, practical tools, and ongoing support to help you manage your diabetes with confidence and improve your overall health and well-being.

We offer one-on-one counseling, virtual support, remote monitoring, and guidance on starting and using diabetes technology, including troubleshooting assistance and ongoing follow-up care.

Our program also offers diabetes education classes, support programs, and special events tailored to people with type 1, type 2, gestational, and less common forms of diabetes.

Behavioral Health Resources and Stress Support

Living with diabetes can affect how you feel physically and emotionally. You may need help adjusting to routines, staying motivated, or coping with stress. Your care team shares reliable education materials and teaches skills that help you manage daily challenges.

When you need support with stress, mood, or motivation, you can connect with our behavioral health providers for counseling or other tools that fit into your care plan.

Healthy Weight Management

Healthy weight management can support better blood sugar control, insulin sensitivity, and overall health in people with diabetes. We focus on sustainable lifestyle changes in nutrition, activity, and behavior, tailored to individual needs.

We collaborate closely with our registered dieticians and lifestyle health specialists to provide coordinated, ongoing support to help patients reach their health goals.

Inpatient Diabetes Care

Lahey’s Inpatient Diabetes Care and Transitional Care Diabetes Service provides specialized care for patients with diabetes during hospitalization. Our multidisciplinary team works together to safely manage blood sugar, support complex insulin needs and address advanced diabetes technologies, including insulin pumps and continuous glucose monitors.

Our care team includes advanced practice clinicians, doctors, nurses, pharmacists, and dietitians who are trained in diabetes care. We work together to make sure each patient gets the right treatment and support.

Goals for Care

No matter what brings you to the hospital or emergency room, your care team will help manage your diabetes and ensure safe and healthy blood sugar levels during your stay.

We care for patients with complex diabetes needs, including those who use insulin pumps, have hard-to-control blood sugars or need complex insulin treatment plans. Our inpatient diabetes team works closely with the hospital care team to manage these challenges safely during the hospital stay.

We also use innovative approaches to improve diabetes care, including research on the use of continuous glucose monitors (CGMs) in the hospital. Our goal is to improve blood sugar control, reduce complications and provide safe, high-quality care for patients with diabetes.

Diabetes Education and Preparing to Go Home

Lahey has a dedicated inpatient diabetes care and education specialist who meets with patients and families during their hospital stay. Education is based on your needs and may include:

  • Checking blood sugar
  • Taking insulin safely
  • Using insulin pens, glucose meters or insulin pumps
  • Understanding your diabetes care plan

Before leaving the hospital, we make sure you feel comfortable managing your diabetes and know what to do next.

Care After You Leave the Hospital

Our inpatient diabetes team coordinates your care and ensures a smooth transition from the hospital through our Transitional Care Clinic, where we address immediate questions or challenges and help set you up for long-term success.

We also work with your primary care provider or diabetes specialist and can help connect you with one if needed. Before you go home, we schedule follow-up visits to ensure you continue to receive the care and support you need.

Diabetes Champions Committee

The Diabetes Champions Committee is a group of nurses, dietitians, pharmacists, doctors and advanced practitioners who care deeply about diabetes care at Lahey.

This group meets every month to share information and improve the way we care for patients with diabetes. Some of their recent initiatives include:

  • Making insulin pump use safer during hospital stays
  • Improving how and when blood sugars are checked
  • Reducing low blood sugar events
  • Improving care for diabetic ketoacidosis (DKA)
  • Helping bedside nurses learn more about diabetes tools like insulin pens, glucose meters and insulin pumps

Additional Services

We work closely with a multidisciplinary team of specialists to provide coordinated, whole-person diabetes care across all settings.

Division of Endocrinology

The Division of Endocrinology, part of the Department of Medicine, offers exceptional patient care, research programs, and education and training opportunities.
